Why South Asians Need Different BMI Standards

The standard WHO BMI thresholds were derived largely from studies involving populations of European descent. When applied to South Asians, they consistently underestimate metabolic risk. Here is why.

Higher Body Fat at Lower BMI

Research from India, Bangladesh and Pakistan shows that South Asians carry 3โ€“5% more body fat than Europeans at the same BMI. This means a South Asian person classified as "normal weight" by WHO standards may already carry excess adipose tissue associated with metabolic disease.

Visceral Fat & Cardiovascular Risk

South Asians have a greater propensity for abdominal visceral fat accumulation โ€” the type of fat stored around internal organs. Visceral fat is metabolically active and strongly linked to insulin resistance, hypertension, dyslipidaemia, and cardiovascular disease, even in individuals with a normal BMI by WHO standards.

Insulin Resistance & Type 2 Diabetes

South Asians develop type 2 diabetes at much lower body weights and at younger ages than most other ethnic groups. Studies from India show that the incidence of diabetes starts rising significantly above a BMI of 23, which is why the South Asian overweight threshold is set lower than the WHO standard of 25.

Genetic & Metabolic Differences

Genetic factors including variants in the TCF7L2 gene and differences in adiponectin levels contribute to the elevated metabolic risk seen in South Asian populations at lower BMI values. These differences are independent of lifestyle and diet, making ethnicity-specific BMI classification clinically essential.

Smaller Frame, Different Proportions

South Asians generally have smaller skeletal frames and shorter leg-to-body ratios compared to Northern Europeans. This means that at any given height and weight, body composition โ€” particularly fat distribution โ€” differs substantially from the reference populations used to establish standard WHO cutoffs.

Clinical Evidence & Official Recognition

The WHO, the International Obesity Task Force, the International Diabetes Federation, and the Diabetes Foundation of India all recognise and recommend lower BMI thresholds for South Asians. The Asia Pacific BMI guidelines (2000) and the 2009 South Asian consensus statement formalised these adjusted cutoffs for clinical practice.

While both systems use the same overweight threshold of BMI 23, South Asian guidelines recommend an even lower obesity threshold of 25 rather than 27.5 used in the broader Asia Pacific guidelines.
